Have the following documents ready before submitting your request form.

The application must include:

  • A copy of the relevant court order and any current parenting plan.
  • Evidence that the parties have attempted to agree on the appointment of a PC (such as correspondence or meeting notes).
  • Any resignation letter(s) from previous mediators or Parenting Coordinators, if applicable.

Important things to note:
PLEASE TAKE NOTE:

1. Should the co-chair only receive one form, FAMAC will appoint a PC / Mediator based on the information provided.

2. A PC can only be appointed if there is a court order in place, and the court order authorises FAMAC to appoint a PC.

3. Where previously appointed PCs have resigned, FAMAC may, in its discretion, decline to appoint a PC if justifiable circumstances exist.

4. If the form is not completed in full, FAMAC reserves the right not to appoint a PC / mediator.

5. FAMAC accredited PCs and/or mediators charge a professional fee which may be up to R 3,000.00 per hour (excluding VAT), depending on the profession and experience of the PC. By requesting the appointment of a PC or mediator, the parties confirm that they are aware of the general fee structure of PCs and mediators, and they agree that a PC or mediator may be appointed.

6. While all attempts will be made to appoint a PC who works in the general geographical area where one or both of the parties resides, this is not always possible.

7. It is important to note that FAMAC's role will be limited to the appointment of a PC / mediator, and that a contractual relationship will then arise between the parties and the PC / mediator.
